If your insurance is not listed, I am able to provide you with a superbill for cash pay visits to submit to your insurance for reimbursement if you have Out of Network benefits.

Please note: I am not able to bill Oregon Health Plan (OHP/CareOregon), even when administered through the above insurance companies.

Also, I am only able to bill select Medicare Advantage plans administered through the above Insurance companies. I cannot bill many Medicare plans. Please double check with your plan to ensure I am in network.
